Umbilical Cord Preservation
The blood contained in the placenta and umbilical cord is a material rich in hematopoietic stem cells capable of generating normal blood cells (red blood cells, white blood cells, and platelets) and represents a valid alternative to bone marrow for stem cell transplantation in both pediatric and adult patients. Stem cell transplantation is the only chance of recovery for many patients affected by leukemia or other serious hematological diseases. Unfortunately, many of these patients do not have a compatible bone marrow donor either among their relatives or in the International Bone Marrow Donor Registries. For these people, umbilical cord blood represents the only hope of being able to undergo transplantation. If it is not donated, umbilical cord blood is usually discarded immediately after birth.
On the day of the appointment, you must go to the DONOR ROOM at Isola Tiberina Hospital, bringing with you:
blood chemistry tests and instrumental examinations carried out during pregnancy;
identity document and tax code/health insurance card for both parents;
Umbilical cord blood can be collected through a simple procedure that involves no risk to either the mother or the newborn. This collection takes place in just a few minutes, is painless, and does not involve any change to normal childbirth care. After birth, once the umbilical cord has been cut, an experienced operator collects the blood remaining in the umbilical cord and placenta in a special bag. The bag is immediately transported to the Cord Blood Bank to be processed and then stored in dedicated freezers. At a public bank it is possible to collect and store umbilical cord blood donations for allogeneic, solidarity-based use, that is, for a patient who does not have a compatible family donor. In special cases, the bank may store cord blood for dedicated use. In fact, if a family member is affected by a disease treatable with transplantation, or if there is a high risk in the family of genetic diseases that could affect future children, the cord blood is stored for the sick family member or for the child himself/herself (dedicated allogeneic or autologous use). These services, upon presentation of the clinical documentation at the time of the appointment, are offered by the Banks at no cost to the patient and his or her family.
The Umbilical Cord Bank of Isola Tiberina Hospital operates in collaboration with the UNICATT Umbilical Cord Bank of the Fondazione Policlinico Gemelli with the aim of helping to build an inventory of umbilical cord blood units for patients affected by diseases treatable with hematopoietic stem cell transplantation but lacking a compatible family donor (allogeneic donation). It supports and provides counseling activities for all those patients and their families for whom dedicated allogeneic or autologous donation is indicated. If umbilical cord blood units cannot be used for transplant purposes, they may be used for the production of other blood components (red blood cells, platelet-rich plasma, platelet gel) both for clinical use and within experimental protocols.
The first umbilical cord blood transplant was performed in 1988 in a patient affected by Fanconi anemia, a severe form of hereditary congenital pancytopenia. The patient received a transplant using a unit collected at birth from the umbilical cord of the patient’s sister, who was found not to be affected by Fanconi anemia and to be HLA-compatible.
Thanks to this first transplant, approximately 60,000 transplants using an umbilical cord blood unit have been performed to date, and more than 800,000 units are stored in about 100 banks worldwide (www.wmda.org). The use of an umbilical cord blood unit has undoubtedly been encouraged by some peculiar features of this type of donation, including the absence of risks for both mother and child, the ease of collection, the possibility of creating an inventory of units available for transplant procedures, the reduced incidence of infections transmitted through transplantation, and prompt availability for clinical use.
